29 CFR 1904.39(a)(2): Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, the employer did not report the in-patient hospitalization, amputation, or loss of an eye to OSHA. (a) 1915 Ebberts Spg CT, Greencastle, PA - An employee sustained an amputation to the right ring finger tip while moving a roller on August 2, 2022 at 11:30 am, and the employer did not report the amputation to OSHA until August 9, 2022 at 1:12 pm. No abatement, certification, or documentation required.
