1910.147(c)(7)(i)(A) Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control 50 College Street, Skinner Green, South Hadley, MA: On or about January 18, 2023, employees working inside a four foot deep excavation were not removed from the excavation when evidence was present of hazardous conditions posed by a steam line that had leaked for some time into the soil and created both thermal and unstable soil condition.
