29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition. Employees were engaged in manufacturing activities at a commercial bakery building located at 1310 Fort Crook Rd N, Bellevue, NE 68005, and were exposed to slip, trip and fall injuries in that the employees were conducting bagging operations while stepping through and over mounds of bread and bags.

Employees were engaged in manufacturing activities at a commercial bakery building located at 1310 Fort Crook Rd N, Bellevue, NE 68005, and were exposed to amputation hazards in that the employees were conducting dough forming operations with their hands near running machinery and were not provided with clear and specific techniques to control hazardous energies.
