29 CFR  1910.30(a)(1): (a) Fall hazards. (1) Before any employee is exposed to a fall hazard, the employer must provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart.  Employers must ensure employees are trained in the requirements of this paragraph on or before May 17, 2017.   a. Component Hardware Group located at 6732 Shook Road, Columbus, OH 43217: On or about January 26, 2023, the employer did not ensure that employees were trained on use of personal fall protection/prevention system while performing order picking and cleaning activities with the use of stock pickers. Employees worked at heights of over five feet to over 15 feet.  Employees were not trained on fall hazards and how to recognize/minimize them in the performance of their duties. The following condition exposed employees to fall hazards of over 15 feet to the concrete ground below.

29 CFR  1910.132(d)(1):The employer shall assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE).  a. Component Hardware Group located at 6732 Shook Road, Columbus, OH 43217, in the powered industrial equipment charging area: On or about January 26, 2023,  the employer did not perform, document and certify the workplace hazard assessment required by this part. As a result, employees were not provided with or required to use personal protective equipment to protect against chemical hazards. Employees performed duties that included forklift battery maintenance which involved exposure to splashed battery acid.

Component Hardware Group located at 6732 Shook Road, Columbus, OH 43217: On or about January 26, 2023, the employer did to require the use of face and eye protection to protect against chemical liquid hazards such as battery acid. Employees perform required duties that included forklift battery maintenance which involved  exposure to splashed battery acid.

29 CFR  1910.305(a)(2)(x): Flexible cords and cables shall be protected from accidental damage, as might be caused, for example, by sharp corners, projections, and doorways or other pinch points.   a. Component Hardware Group located at 6732 Shook Road, Columbus, OH 43217, located in the assembly area: On or about January 26, 2023, the employer did not ensure that the flexible portable power cord used to provide power to the Better Pack Model 555eS tape dispenser machine was protected from accidental damage. The flexible portable power cord was in an area with sharp corners, manual pallet jack traffic and other pinch points resulting in damage to the outer insulated cover of the cord exposing internal materials. The following condition exposed employees to electrical shock hazards of up to 115 volts.

29 CFR  1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) shall be visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket). Cord and plug connected equipment and flexible cord sets (extension cords) which remain connected once they are put in place and are not exposed to damage need not be visually inspected until they are relocated.   a. Component Hardware Group located at 6732 Shook Road, Columbus, OH 43217, located in the assembly area: On or about January 26, 2023,  the employer did not ensure cord and plug connected equipment, which were exposed to damage, were inspected for external defects. Employees utilized a damaged flexible cord to power the Better Pack Model 555eS tape dispenser machine.  The  flexible cord  was damaged where the outer insulating jacket was torn  exposing the internal materials, exposing employees to a 115-volt electrical contact hazard.
