29 CFR 1926.102(a)(1): Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: a) At 813 Eva Kennedy Drive, Lot 64: On or about January 26, 2023, workers were observed exposed to struck-by and eye injury hazards while performing roofing operations in that the employer did not enforce the wearing of eye protection while using equipment such as but not limited to pneumatic nail guns.

29 CFR 1926.502(d)(17):The attachment point of the body belt was not located in the center of the wearer's back. The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head. a) At 813 Eva Kennedy Drive, Lot 64: On or about January 26, 2023, workers were observed exposed to approximately 21-foot fall hazards from the 6/12 pitched roof in that the employer allowed the fall protection connection to be at the rock climbing harness and not a full body harness with the lanyard connection at the wearer's back.
