29 CFR 1926.451(c)(2)(iv):Front-end loaders and similar pieces of equipment shall not be used to support scaffold platforms without being specifically designed by the manufacturer for such use. On or about January 24, 2023, an employee used a field designed wooden platform on a GEHL RS5-34 Telescopic lift in excess of 20 feet high as a scaffold platform for personnel residential roofing activities. The GEHL RS5-34 Telescopic lift was not designed to be used to lift personnel without the machine being equipped with personnel work platform safety system. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. On or about January 24, 2023, Employees were exposed to fall hazards in excess of 20 feet while engaged in residential roofing activities. Fall protection was not being utilized while employees were engaged in roof installation activities. No abatement certification or documentation is required for this item. Felipe's Roofing & Home Improvement Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1587696, citation number 1 , item number 2 and was affirmed as a final order on October 7, 2022, with respect to a workplace located at 926 Greenwood Ave., Wilmette, IL. 60091.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grab rail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support. On or about January 24, 2023, the portable ladder used to access the roof level did not extend at least three (3') feet above the upper landing surface, thereby exposing employees to fall hazards in excess of 15 feet. No abatement certification or documentation is required for this item. Felipe's Roofing & Home Improvement Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1587696, citation number 1 , item number 4 and was affirmed as a final order on October 7, 2022, with respect to a workplace located at 926 Greenwood Ave., Wilmette, IL. 60091.
