29 CFR 1910.28(b)(1)(i):29 CFR 1910.28(b)(1)(i): Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i): Employees engaged in maintenance operations at A.C. Nelsen RV World located at 11818 L Street, Omaha, Nebraska 68137, were exposed to fall hazards in that the RV techs were accessing the top of RV's to conduct inspections and maintenance repairs on RV's at heights greater than 4 feet without any form fall protection.

29 CFR 1910.178(a)(4):Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly. Employees engaged in forklift operations at A.C. Nelsen RV World located at 11818 L Street, Omaha, Nebraska 68137, were exposed to crushing and struck-by hazards in that employees were using two forklift attachments that were not pre-approved by the manufacturer. Employees were using the forklift attachments to move RV's in and out of the RV bays and around the property.

Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.
