29 CFR 1910.23(c)(4): The employer did not ensure that ladders that are used only on stable and level surfaces unless they are secured or stabilized to prevent accidental displacement. United Methodist Homes, 584 Long Hill Avenue, Shelton, CT 06484, Front of the building drop off area.: On or about January 20, 2023 an employee was tasked with removing a Christmas wreath from the outside of the building entrance. The ladder slipped and the employee fell approximately 12 feet sustaining a serious injury where as the ladder was placed on a slippery surface.

United Methodist homes, 584 Long Hill Ave Shelton, CT 06484; Right in front of the pick up area. On or about January 20, 2023, an employee was working on a ladder where it was located directly in front of a building entrance / exit and was not either secured or guarded.
