1910.261(b)(1):  The employer did not ensure that the main power disconnect switch or valve, or both, controlling its source of power or flow of material, was locked out or blocked off with padlock, blank flange, or similar device before conducting any maintenance, inspection, cleaning, adjusting, or servicing of equipment (electrical, mechanical, or other) that required entrance into or close contact with the machinery or equipment:  (a) Site located at 437 Old Augusta Road South, Rincon, GA 31326: On or about January 19, 2023, the employer exposed employees to burn hazards in that the tertiary fine screen filter basket on Paper Machine-19 was not locked out nor was the stored energy released before maintenance was conducted.
