29 CFR 1910.216(b)(1)(iii): Safety tripwire cables installed on the front and back of the mill were more than 72 inches above the level on which the operator stands. a. BRP Manufacturing Company, 637 North Jackson Street, Lima, Ohio 45801: On or about January 30, 2023, the employer did not ensure that the tripwire cable, located on the inbound side of Banbury Mix Mill #9, was no more than 72 inches above the level where the operator stands. The tripwire cable was position behind the front of the roll and at a height of approximately 83 inches from the standing surface, impeding prompt stoppage of the machine in the event of an emergency.

BRP Manufacturing Company, 637 North Jackson Street, Lima, Ohio 45801: On or about January 30, 2023, the employer failed to ensure that the OSHA Form 300 for calendar year 2023 contained a complete description in column F for multiple entries. The 2023 300 log did not define the substance that directly injured or made person ill. b. BRP Manufacturing Company, 637 North Jackson Street, Lima, Ohio 45801: On or about January 30, 2023, the employer failed to ensure that the OSHA Form 300 for calendar year 2022 contained a complete description in column F for multiple entries. The 2022 300 log did not describe the injury or illness.
