BRP Manufacturing Company, 637 North Jackson Street, Lima, Ohio 45801: On or about January 30, 2023, the employer did not ensure that the tripwire cable, located on the inbound side of Banbury Mix Mill #9, was no more than 72 inches above the level where the operator stands. The tripwire cable was position behind the front of the roll and at a height of approximately 83 inches from the standing surface, impeding prompt stoppage of the machine in the event of an emergency.
