29 CFR  1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest system.   (a) On or about 1/25/2023, 1122 King St, Christiansted, St Croix 00820 - Employees were working on the roof with an unprotected side or edge approximately 20 feet (6.1 m) in height, exposing employees to a fall hazard.  NOTE: Certification and documentation of abatement must be submitted for this citation item as required by CFR 1903.19(c).

29 CFR  1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, nor was it designed, installed, and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person.  (a) On or about 1/25/2023, 1122 King St, Christiansted, St Croix 00820 - Employees identified an anchor to connect and use as a personal arrest that was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, nor was it designed, installed, and used as part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person. Exposing employees to a fall hazard.  NOTE: Certification and documentation of abatement must be submitted for this citation item as required by CFR 1903.19(c).

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface, and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) On or about 1/25/2023, 1122 King St, Christiansted, St Croix 00820 - employees used an aluminum portable ladder (14 Ft) (6) to gain access to the upper level of the single-pole wooden scaffold, and it did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access, and the ladder was not secured at its top to a rigid structure, exposing employees to fall from a ladder.  NOTE: Certification of abatement is not required for this citation item as abatement was documented by the compliance safety and health officer during the inspection.
