OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the employees were exposed to fall and struck by hazards.  a)  On or about  January 25, 2023, at a jobsite located at 3231 N Cicero Ave., Chicago, IL 60641, the employees were utilizing a XIAOJINGANG T-Series Electric Hoist Winch as part of an installation of audio and visual effects equipment.  An employee was attached to the winch via a safety climber's harness and was hoisted into the air, which exposed the employee to a fall hazard of approximately 30 feet to the ground below.  No abatement certification or documentation is required for this item.
