Himes Ave Tampa, FL: On or about January 31, 2023 the employer exposed employees to serious eye injuries in that the employer did not ensure that each employee used appropriate eye or face protection when using caustic liquid chemicals such as Smart/Wash� Angry Orange, Dakils D-256, and WYSIWASH Jacketed Caplets to clean and sanitize areas throughout the facility.

The employer did not provide hazard assessment documentation for job tasks that require the use of chemical, such as but not limited to Smart/Wash� Angry Orange, WYSIWASH Jacketed Caplets, and Dakils D-256 to disinfect and sanitize areas throughout the facility.

Himes Ave Tampa, FL: On or about January 31, 2023 the employer exposed employees to serious eye and skin injuries, in that the employer did not provide training on the use of Personal Protective Equipment (PPE) such as but not limited to gloves, safety glasses or goggles, and protective clothing for liquid chemicals such as Smart/Wash� Angry Orange, Dakils D-256, and WYSIWASH Jacketed Caplets. These caustic chemicals are used to clean and sanitize areas throughout the facility.
