OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish a place of employment which was free from recognized hazards that were likely to cause death or serious physical harm to employees exposed to crushing type injuries, due to the unsafe operation of defective automotive lifts:  Service Area - Auto lifts were operated with defects, in the following instances:  a) Lift #1 - Model #SPO18N310 - Smooth hydraulic cylinder operation and/or hydraulic cylinder leaks. b) Lift #4 - Model #KPH370-42YKU - Smooth hydraulic cylinder operation and/or hydraulic cylinder leaks, leaks on hoses/pipes/fittings and/or improper routing of hoses/pipes/fittings. c) Lift #5 - Model #KPH370-42YKU - Smooth hydraulic cylinder operation and/or hydraulic cylinder leaks. d) Lift #6 - Model #SPOA10N700 - Leaks on hoses, pipes, fittings, and/or improper routing of hoses/pipes/fittings. e) Lift #7 - Model #RX9GM9000 - Snap ring needed on bottom pin, defective jacks, labeling.   f) Lift #8 - Model #SPOA10N800 - Defective adapters and arms. g) Lift #9 - Model #SPOA10N700 - Electrical wiring and components are defective. h) Lift #10 - Model #SPO10N700 - Leaks on hoses, pipes, fittings, and/or improper routing of hoses/pipes/fittings. i) Lift #11 - Model #SPOA9N400 - Leaks on hoses, pipes, fittings, and/or improper routing of hoses/pipes/fittings, labeling .  j) Lift #13 - Model #KPH370-42YKU - Defective hydraulic cylinder and/or hydraulic cylinder leaks.  Leaks on hoses, pipes, fittings and/or improper routing of hoses/pipes/fittings. k) Lift #14 - Model #KPH370-42YKU - Defective hydraulic cylinder and/or hydraulic cylinder leaks.  Defective stop bolts and spring on left side.
