Jobsite: 540 Church Rd., Avondale, PA 19311-An employee was exposed to crushing and caught-in-between hazards while operating and/or working in close proximity to the chute at the rear of the Mack truck in that the chain & sprocket assembly was not enclosed or guarded.  a) Left side of the chute (Sprocket spread - approx. 16.5 inches) b) Right side of the chute (pulley edge to pulley edge - approx. 19.5 inches, large pulley - approx. 9.5 inches, small pulley on electric motor - approx. 3.75 inches)   Condition noted on/about January 27, 2023

29 CFR  1928.57(c)(2)(ii): All revolving shafts, including projections such as bolts, keys, or set screws, were not guarded.  Jobsite: 540 Church Rd., Avondale, PA 19311-Employees were exposed to entanglement hazards while operating and/or working in close proximity to revolving and projecting horizontal shafts that were not guarded.  a) The revolving horizontal projecting shaft measuring approximately 2" in length and 1.25" in diameter located below the framework of the Casing Conveyor. Condition noted on/about January 27, 2023  b) The revolving horizontal shaft measuring approximately 90 inches in length and 2.25" in diameter, and projected out approximately 2.75 inches on the left side and approximately 2 inches on the right side, and is located at the bottom of the Peat Moss Dump Conveyor.    Condition noted on/about January 24, 2023.

OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards.  a) Jobsite located at 550 Church Road, Avondale, PA 19311, Mushroom House Number 8 -On or about January 27, 2023, employees performed tasks associated with mushroom growing activities such as, but not limited to harvesting mushrooms, watering mushroom beds, and cleaning the mushroom house from the second level of the Mushroom House. Employees performed these tasks from unsecured and movable walking/working surfaces located 17 inches away from the mushroom beds.  Employees were not provided with a means of fall protection exposing employees to falls up to 93 inches to the lower level.   An acceptable means of abatement would be to fully plank and secure the walking/working surfaces and/or ensure that a fall protection system (guard rails, covers, nets or personal fall arrest system) be in place for employees working from elevated walking/working surfaces.

Among other methods, some feasible and acceptable means of hazard abatement include but are not limited to:   Follow National Fire Protection Association's National Electrical Code for Receptacles in Damp and Wet Locations NFPA70 406.9(b)(1).

Among other methods, some feasible and acceptable means of hazard abatement include but are not limited to:   1. Follow the 2017 NEC edition of NFPA 70, 2014 Code Language: 406.4(D)(2) Non-Grounding-Type Receptacles. 2. Follow the National Fire Protection Association - NFPA 70E Standards for Electrical Safety in the Workplace .
