29 CFR  1910.303(b)(1): Examination. Electric equipment shall be free from recognized hazards that are likely to cause death or serious physical harm to employees. Safety of equipment shall be determined using the following considerations:   (ii): Mechanical strength and durability, including, for parts designed to enclose and protect other equipment, the adequacy of the protection thus provided;   a.) On or about January 30, 2023, the employer did not ensure the grounding cable for the building was protected to prevent damage to the cable located on the ground from being damaged by equipment including lawn mowers.

29 CFR  1910.304(g)(5):  Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent, continuous, and effective.  a.) On or about January 30, 2023, the path to ground was not permanent and continuous in that it was severed and did not prevent an electrical charge from igniting the hydrogen gas produced during the reaction process.
