29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) On or about February 1, 2023, employees were exposed to laceration/amputation hazards while working on and around the Chop Saw (Model No. MTS18 & Serial No. 102220-MTS). The interlock on the saw had been overridden with a wedge block of wood.

29 CFR 1910.303(b)(7) Mechanical execution of work. Electric equipment shall be installed in a neat and workmanlike manner. (a) On or about February 1, 2023, employees working on Bandsaw One (Model 64" & Serial No. 060706-642M) were exposed to an electrical shock hazard. The energized wires were not properly guarded, in-that the energized wires were only guarded with wire nuts and the insulation had pulled away from the live wires. (b) On or about February 1, 2023, employees working on Bandsaw Two (Model 64" & Serial No. 060806-642M) were exposed to an electrical shock hazard. The energized wires were not properly guarded, in-that the energized wires had the insulation pulled away from the live wires.
