29 CFR 1910.269(d)(2)(i):The employer did not establish a program consisting of energy control procedures, employee training, and periodic inspections to ensure that, before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, start up, or release of stored energy could occur and cause injury, the machine or equipment is isolated from the energy source and rendered inoperative. a) Haviland Energy LLC on or about January 23, 2023, at the address of 151 West Main Street, Paulding, OH.: The employer failed to implement an effective energy control program consisting of control procedures, employee training and periodic inspections. Employees performing service/maintenance duties on the 800KW power generation unit and associated fuel lines without the benefit of energy isolation, procedures, adequate training and periodic inspections, were exposed to burn hazards created by energized fuel lines.

The employer or employer representative failed to contact the Toledo Area Office or the 24-hour OSHA number to notify the agency of employees' in-patent hospitalization within 24 hours.
