29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) 1321 Plaza Road Fair Lawn, NJ: CSHOs observed an employee engaged in roofing activities on a 8on 12 pitched roof in which roofing bundles were being staged  overhead employees exposing the employees to overhead impact hazards as head protection was not utilized, on or about 2/1/2023.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) 1321 Plaza Road Fair Lawn, NJ: Employees engaged in roofing activities on a 8 on 12 pitch roof were exposed to fall hazards of approximately 18ft to the ground below in which a portable extension ladder used to access the roof, the ladder did  not extend 3ft above the upper landing surface, observed on or about 2/01/2023.
