OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and crush-by hazards when utilizing damaged steel storage racks:    a)  Warehouse 1, on or about 02/01/2023, throughout the warehouse the employer did not ensure that the steel storage racks with visible damage were immediately isolated until the damaged portions were evaluated by a storage rack design professional and were not placed back into service until repaired and certification was obtained that the repaired components of the storage rack system were restored to at least their original design capacity.  b) Warehouse 2, on or about 02/01/2023, throughout the warehouse the employer did not ensure that the steel storage racks with visible damage were immediately isolated until the damaged portions were evaluated by a storage rack design professional and were not placed back into service until repaired and certification was obtained that the repaired components of the storage rack system were restored to at least their original design capacity.
