29 CFR 1910.22(a)(3): The employer did not ensure that walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice.  Employees engaged in maintenance/housekeeping activities in the chemical and equipment rooms of the facility were exposed to tripping hazards in the following instances: 1. Equipment Room - Employees engaged in maintenance/housekeeping activities exposed to tripping hazards due to a four inch (4 in.) diameter hole in the grating of the center isle of the walking/working surface. 2. Chemical Room - Employee engaged in maintenance/housekeeping activities exposed to tripping hazards due to a four inch by six inch (4 in. x 6 in.) hole in the grating of the walking/working surface.

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  Employees engaged in maintenance activities in the vacuum house/room of the facility are exposed to caught-in and in-going nip point hazards. The rotating shafts on the vacuum turbine motors were not provided with machine guarding to prevent accidental contact.

The drive pulleys on the  double stack pumping station units were not provided with guards on the back of the machine. In-going nip points were created by two exposed 14 inch pulleys and associated belts on the back of the machine.

Employees engaged in maintenance/housekeeping activities in the equipment room were exposed to electrical shock hazards in that a flexible cord is used as a substitute for fixed wiring to power an automatic moisture relief valve. Employees conduct housekeeping activities adjacent to the air compressor area where a housekeeping washer appliance is operated.

29 CFR  1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  Employees engaged in maintenance/housekeeping activities were exposed to chemical hazards in that they were not provided training on the chemical hazards of Lustra shampoos, detergents, polishes and presoaks at the time of their initial assignment.  The chemical hazards associated with Lustra shampoos, detergents, polishes and presoaks include: severe skin burns and serious eye damage, throat irritation, abdominal pain, nausea and vomiting, vertigo, loss of consciousness, is corrosive to mucous membranes and upper respiratory tract and may cause cancer and damage to organs. Additionally, these Lustra products can act as a fueling agent as they are noted as combustible liquids.
