To include, but not limited to: -Truck safety related topics including the use of operator seat belt and workplace related topics; -Evaluation of the operator's performance in the workplace. Condition noted on or about 9/28/2022.

The employer failed to provide documentation which included the name of the operator, the date of the training, that date of evaluation, and the identity of the person(s) performing the training.
