29 CFR  1910.147(f)(3)(ii)(D):Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment:  (a) Production Area; On or about April 4, 2023, and time prior to, the employer did not ensure that lock out devices were utilized by each authorized employee while performing repairs/changes to the Double Head Saw.  Only one lock was applied and the key was left in the lock.

29 CFR 1910.178(n)(4): The driver shall be required to slow down and sound the horn at cross aisles and other locations where vision is obstructed. If the load being carried obstructs forward view, the driver shall be required to travel with the load trailing.  (a) Warehouse Area; On or about February 1, 2023 and at times prior to, employees walking  the aisle between the exit door and the warehouse were exposed to struck-by hazards, in that the employer did not ensure that forklift drivers slowed down and sounded the horn to protect workers from forklift traffic.  One of the forklifts horn was  inoperable.

29 CFR  1910.213(r)(4): Suitable guard(s) were not provided to reduce to a minimum the hazard due to the point of operation of rotating blades(s):   (a) Production Area; On or about February 1, 2023, and times prior to, the employer failed to provide machine guarding for the Double Head  Chop saw exposing employees to amputation hazards in that it was not guarded to prevent accidental contact with the point of contact of rotating blades.  (b)Saw Area; On or about February 1, 2023,  and times prior to, the employer failed to provide machine guarding for the Baker Single Head Saw, exposing employees to amputation hazards in that it was not guarded to prevent accidental contact with the point of operation.  (c)Production Area; On or about February 1, 2023,  and times prior to, the employer failed to provide machine guarding for the Pop Up Trim Saw, exposing employees to amputation hazards in that it was not guarded to prevent accidental contact with the point of operation of rotating blade.
