29 CFR 1910.178(n)(4): The driver shall be required to slow down and sound the horn at cross aisles and other locations where vision is obstructed. If the load being carried obstructs forward view, the driver shall be required to travel with the load trailing. (a) Warehouse Area; On or about February 1, 2023 and at times prior to, employees walking the aisle between the exit door and the warehouse were exposed to struck-by hazards, in that the employer did not ensure that forklift drivers slowed down and sounded the horn to protect workers from forklift traffic. One of the forklifts horn was inoperable.

(c)Production Area; On or about February 1, 2023, and times prior to, the employer failed to provide machine guarding for the Pop Up Trim Saw, exposing employees to amputation hazards in that it was not guarded to prevent accidental contact with the point of operation of rotating blade.
