The employer did not ensure that eye and face protection was worn by affected employees to protect against flying particles, on or about 2/2/2023.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) 502 Terrace Ave Hasbrouck Height, NJ: Employees were engaged in roofing activities on a 8 on 12 pitch roof were exposed to fall hazards of approximately 40ft to the ground below in which personal fall arrest harnesses were not being utilized and not secured to an anchor point to prevent fall hazards. Condition observed on or about 02/02/2023.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a)502 Terrace Ave Hasbrouck Height, NJ: Employees engaged in roofing activities on a 8 on 12 pitch roof were exposed to fall hazards of approximately 40ft to the ground below in which a portable extension ladder used for access whose ladder side rails did not extend 3ft above the upper landing surface. Condition observed on or about  2/2/2023.
