At the Front Canopy/Roof area of the facility located at 1520 Edgewood Avenue, North, Jacksonville, Florida 32254: On or about February 1, 2023, the employer exposed two employees to a 12-foot, 4-inch fall hazard, in that fall protection was not utilized by employees while on the canopy/roof to remove vegetation.

At or near the Front Canopy/Roof area of the facility located at 1520 Edgewood Avenue, North, Jacksonville, Florida 32254: On or about February 1, 2023, the employer exposed two employees to fall hazards ranging from 4 to 12-foot, 4-inch, in that body harnesses with lanyards were not utilized by employees while working from, being elevated or lowered by an aerial lift.
