29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration; such activities were not performed only by experienced and trained employees selected for such work by the competent person:  (a) On or before 2/2/2023, the employer did not ensure that scaffolds were erected under the supervision of a competent person qualified in the erection, moving, dismantling, alteration of scaffolds. This condition exposed employees to scaffold collapse and fall hazards.

29 CFR 1926.452(c)(2): All cross braces were not of such length so as to automatically square and align vertical members such that the erected scaffold was always plumb, level, and square:  (a) On or before 2/2/2023, the employer did not ensure that scaffold cross members fit in such a way as to make the vertical members square and plumb. The cross braces were too long, were secured with wire and not pinned to the scaffold uprights as required by this standard. This condition exposed employees to scaffold collapse and fall hazards.
