29 CFR  1910.28(b)(1)(i):  Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  a) Mezzanine Area, 470 Industrial Park Road, Ebensburg, PA 15931:  Observed on or about February 2, 2023, the employer did not ensure that each employee working on the second, third, and fourth levels of the mezzanine were protected from falling through the use of guardrail systems, safety net systems, or personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems.   The walking-working surfaces that were even with the wire mesh racking were not equipped with an intermediate or midrail.  Employees loading and unloading boxes from the wire mesh racking were exposed to a fall hazard of up to approximately 30 feet (fourth level) to the concrete floor below.

29 CFR  1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet:  a)  Maintenance Shop Area, 470 Industrial Park Road, Ebensburg, PA 15931:  Observed on or about February 2, 2023, an oxygen cylinder (UN1072) was stored directly next to an acetylene cylinder (UN1001) to the left of an Exit door in the maintenance shop.  Employees working in the maintenance shop were exposed to a fire hazard.

29 CFR  1910.305(g)(1)(iv)(B): Flexible cords and/or cables were run through holes in walls, ceilings, or floors:   a)  Maintenance Shop Area, 470 Industrial Park Road, Ebensburg, PA 15931:  Observed on or about February 2, 2023, a flexible cord (110 Volt) used to power heat tape was run through a hole in the wall near a Sterling brand HVAC system.
