29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: (a) 1415 Weavertown Road, Lebanon, Pennsylvania - An employee suffered amputation to the left index and middle fingertips as a result getting pinched in the sealing head of a repack machine on October 28, 2022. The employer did not report the amputation to OSHA within 24 hours. Corrected during the inspection.
