29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Employees engaged in printing press operational activities at the 1047 Blackbird and 1054 Blackbird printing presses were exposed to amputation, caught-in, and in-going nip point hazards. Exposed, driven, rollers throughout the printing press machines were not provided with guards to protect against accidental contact.

29 CFR 1910.219(c)(4)(i): Unguarded projecting shaft end(s) did not present a smooth edge and end and projected more than one half the diameter of the shaft: Employees engaged in printing press operational activities at the 1047 Blackbird printing press were exposed to caught-in and amputation hazards. Employees are exposed to an unguarded two inch (2 in.) diameter rotating shaft end that projected three inches (3 in.) and was not provided with a guard.

The exposed drive pulley at the bottom of the printing press was not provided with a guard at the bottom of the machine. In-going nip points were created by an exposed pulley and associated belt at the bottom of the machine approximately 16 inches (16 in.) from the floor level below.
