29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) Jobsite:  15009 Lake Bessie Loop, Winter Garden, employer's ladder was only extended approximately 2 foot passed the roof landing, while being used it to gain access to the 20 feet roof to install shingles, exposing employees to fall hazards, on or about 2/3/2023.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) Jobsite:  15009 Lake Bessie Loop, Winter Garden: employees were exposed to 20 foot fall hazards while installing shingles on a 6/12 pitched roof, in that, fall protection was not used, on or about 2/03/2023.  Gulf Western Roofing & Sheet Metal was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13) which was contained in OSHA inspection number 1368437, citation number 1, item number 1 and was affirmed as a final order on 09-04-2020, with respect to a workplace located at 8863 Madrid Circle, Naples, Florida 34101.  Gulf Western Roofing & Sheet Metal was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13) which was contained in OSHA inspection number 1412995, citation number 1, item number 1 and was affirmed as a final order on 09/24/2019, with respect to a workplace located at 4304 Barbour Trail, Odessa, Florida 33556.  Gulf Western Roofing & Sheet Metal was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13) which was contained in OSHA inspection number 1313204, citation number 2, item number 1 and was affirmed as a final order on 07-29-2019, with respect to a workplace located at 9255 Bexley Drive, FT. Meyers, Florida 33967.
