29 CFR 1910.305(b)(2)(I): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy: Employees were exposed to electrical shock hazards. Employees working at Dollar General in Baxter Springs, Kansas were exposed to electrical shock hazards from exposed 120 VAC energized conductors in that outlet boxes were not covered in the following locations: a) In the managers office, phone charger plugged into energized outlet not provided a cover or face plate. b) Next to water fountain in back room, energized without a cover or face plate. c) On the interior front wall of the store, a junction box with energized conductors from old automatic doors not provided a cover or face plate.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure: Employees were exposed to electrical shock and fire hazards. Employees working at or near the cash register in the front of the store at Dollar General in Baxter Springs, Kansas were exposed to electrical shock and fire hazards. A single power strip was used to provide 120 VAC power to three portable space heaters.
