29 CFR  1926.1053(b)(7): Care was not exercised in placing, lashing, or holding a ladder with slip-resistant feet that was used on a slippery surface:  (a) 19038 E. Hampton Dr., Aurora CO 80013: On or about February 2, 2023, the employer did not ensure or exercise care in placing an extension ladder on the first story roof of a single-family home that had accumulations of snow and ice on the roof surface.  An employee used an extension ladder to transition from the first to the second story which was not adequately secured and placed on a slippery surface. The ladder slipped when an employee attempted to use it and both the ladder and the employee fell to the ground, causing injury. 29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:   (a)  19038 E. Hampton Dr., Aurora CO 80013: On or about February 3, 2023, and at times prior, the employer did not ensure employees  working at heights greater than 6 feet were protected by a fall protection system.  Two employees were observed working on the first and second story roof without use of a fall protection system.  Freedom Forever Texas LLC, d/b/a Freedom Forever Solar was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1634118, citation number 1, item number 1 and was affirmed as a final order on January 25, 2023, with respect to a workplace located at 2429 Mike Price, El Paso, TX 79938.  Freedom Forever Illinois LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1610413, citation number 1, item number 1 and was affirmed as a final order on October 4, 2022, with respect to a workplace located at 601 Kingsbrooke Crossing, Bolingbrook, IL 60440.  Freedom Forever - San Antonio was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1626761, citation number 1, item number 1 and was affirmed as a final order on January 20, 2023, with respect to a workplace located at 6906 Cozy Lane, San Antonio, TX 78218.  Freedom Forever Texas LLC d/b/a Freedom Forever Solar was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1565326, citation number 1, item number 1 and was affirmed as a final order on February 9, 2022, with respect to a workplace located at 7504 Hitching Post, El Paso, TX 79911.
