Jewell Electric Inc.; 100 Hospital Ave Dubois, Pa 15801- On or about 02/06/2023 and at times prior thereto, An employee was working inside an energized 208V DPEM transfer switch in the basement electrical room. The employee received an electrical shock which required another employee to reach inside the energized switchgear for rescue purposes.

The DPEM Transfer Switch#5 208V electrical Switchgear was not rendered inoperative prior to employees working inside the electrical equipment.

29 CFR  1926.21(b)(2):The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  a.) Jewell Electric Inc.; 100 Hospital Ave Dubois, Pa 15801- On or about 02/06/2023 and at times prior thereto, an employee was working in an energized 208V DPEM transfer switch switchgear. The employee had not been instructed in the electrical safety related work practices and/or the requirements noted in NFPA70E prior to starting the electrical work.  b.) Jewell Electric Inc.; 100 Hospital Ave Dubois, Pa 15801- On or about 02/06/2023 and at times prior thereto, personnel were potentially exposed to electrical hazards while working on, with, and around live electrical equipment and were not trained on the requirements of 29CFR 1926.417 (i.e. Electrical Lockout/Tagout).  c.) Jewell Electric Inc.; 100 Hospital Ave Dubois, Pa 15801- On or about 02/06/2023 and at times prior thereto, and at times prior thereto, personnel were potentially exposed to electrical hazards while working on, with, and around live electrical equipment and were not trained on the requirements of 29CFR 1926.416 (i.e. Safe Work Around Electrical Systems).

Jewell Electric Inc.; 100 Hospital Ave Dubois, Pa 15801- On or about 02/06/2023 and at times prior thereto, an employee was working in the 208V DPEM transfer switch switchgear  while it was energized without electrical personal protective equipment in the basement electrical room. Employees did not have the appropriate arc flash personal protective equipment available to perform the work. Missing personal protective equipment include, but is not limited to; arc flash gloves, face shield, and suit.

Test voltages and the maximum intervals between tests shall be in accordance with Table E-4 and Table E-5.   a.) Jewell Electric Inc.; 100 Hospital Ave Dubois, Pa 15801- On or about 02/06/2023 and at times prior thereto, rubber insulating arc flash gloves available for employee use exceeded the maximum intervals between tests. The gloves available to employees had an inspection date of September 28, 2017.

29 CFR  1926.416(a)(3):The employer began work without determining if employees could contact any part of an energized circuit and did not post warning signs and advise employees of the location of the circuit, hazards, and protective measures they need to take.   a.) Jewell Electric Inc.; 100 Hospital Ave Dubois, Pa 15801- On or about 02/06/2023 and at times prior thereto, an employee was removing electrical components including, but not limited to, fire pump wiring inside the energized 208V DPEM transfer switch switchgear in the basement electrical room. While removing the wires, the employee contacted an unknown energized source inside the 208V switchgear. The entire electrical system was not thoroughly metered with instrumentation to determine whether any part of an energized electric power circuit was present in the area where the employee was working. After the accident, it was determined that terminals were energized inside the DPEM switchgear.
