29 CFR 1910.22(d)(1): The employer did not ensure that walking-working surfaces were inspected, regularly and as necessary, and maintained in a safe condition: a) At the fresh air intake shaft for Building 1330, where the employer did not inspect the metal grating system. Subcontracted employees were exposed to fall hazards of approximately 30-feet while walking atop the unsecured grating of the shaft.

29 CFR 1910.22(b): The employer did not ensure that each walking-working surface could support the maximum intended load for that surface: a) At an outdoor fresh air intake shaft for the air conditioning ventilation system for Building 1330, a landscape vendor employee fell between a concrete ledge and a portion of steel grafting that had been previously installed before SBRE became the property manager.
