29 CFR  1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide:  a)  At 26110 County Road 13 Building A, Jubilee Family Dentistry, Daphne, Alabama:  On February 7, 2023; the employer allowed his employee to work from a scaffold  with a work platform that measured 12 inches wide and not 18 inches wide as required by the standard up to 14 feet 6 inches to the ground below.

29 CFR  1926.451(f)(15): Ladders were used on scaffolds to increase the working level height of employees: without satisfying the criteria of subparagraphs (i) - (iv) of this paragraph:  a)  At 26110 County Road 13 Building A, Jubilee Family Dentistry, Daphne, Alabama:  On February 7, 2023; the employer allowed his employee to work from a 8 foot a-frame ladder set on the scaffold platform to perform framing activities exposing his employee to a fall hazard of up to 14 feet 6 inches to the ground below.
