29 CFR 1926.451(b)(1): Each platform on all working levels was not fully planked or decked between front uprights and the guardrail supports: (a) On or about 02/06/2023 at the site of 46-01 37th Ave, Long Island City, NY Employees engaged in forming a rebar cage were standing on a platform that had only one plank measuring approximately 10 inches wide. The employees were exposed to a fall hazard of approximately 7 ' to the ground.

The rebar cage collapsed.

Other employee was climbing on rebars to gain access to the platform.
