29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary. a) Worksite located at 215 N 9th St Philadelphia, PA 19107: On or about February 7, 2023, the employer did not ensure poles of the pumpjack scaffold were secured to the structure by rigid triangular bracing or equivalent at the bottom and other points as necessary. Employees performed siding work from the unsecured pumpjack scaffold, thereby exposing employees to fall hazards.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor was employee provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) Worksite located at 215 N 9th St Philadelphia, PA 19107: On or about February 7, 2023, the employer did not ensure each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system. Employees were installing siding on a residential construction while standing on the roof of the adjacent first story building without the use of fall protection, thereby exposing employees to fall hazards of approximately 20 to 30 feet.
