On or about February 7, 2023 at the address of 27463 County Rd. 424 Defiance OH, 43512: The employer did not ensure that each powered industrial truck operator successfully completed training prior to permitting employee(s) to operate the rough terrain forklift serial# 6000M1F1031. Without effective training and evaluation the operator, as well as others working in and around forklift were exposed to struck-by hazards.

Employees working in this condition were exposed to potential struck by hazards (flying projectile/ricochet nails).

On or about February 7, 2023 at the address of 27463 County Rd. 424 Defiance OH, 43512: The employer failed to ensure the picks (platform) used to form working platforms (scaffolding) on which employees operate while installing roofing material, was at least 18 inches wide. Employees operating from scaffolding platforms in this condition were exposed to a potential fall of up to 14ft to the ground below.

On or about February 7, 2023, at the address of 27463 County Rd. 424 Defiance OH, 43512: The employer did not provide a training program for each employee task with using extension ladders to access/exit a two-story residential roof structure. A training program would enable each employee to recognize the hazards of falling from the ladder and the procedures to be followed in order to minimize these hazards.
