29 CFR  1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  a. On the roof of a temporary residential worksite, at 1715 McQuade Street, Jacksonville, Florida 32209: On or about February 3, 2023, the employer exposed three employees to eye injury hazards caused by airborne debris and flying objects, in that eye protection was not utilized by the employees, while using pneumatic nail guns to sheath the roof.  b. On the roof of a temporary residential worksite, at 1711 McQuade Street, Jacksonville, Florida 32209: On or about February 3, 2023, the employer exposed two employees to eye injury hazards caused by airborne debris and flying objects, in that eye protection was not utilized by the employees while using saws to cut sheets of plywood and 2 x 4's.  c. On the roof of a temporary residential worksite, at 1711 McQuade Street, Jacksonville, Florida 32209: On or about February 3, 2023, the employer exposed himself to eye injury hazards by airborne debris and flying objects, in that eye protection was not utilized by the employee while using a pneumatic nail gun to sheath the roof.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:   a.  On a 5:12 pitched roof of a temporary residential worksite, at 1715 McQuade Street, in Jacksonville, Florida 32209: On or about February 3, 2023, the employer exposed three employees to fall hazards ranging from 8 feet to 15 feet 3-inches, in that fall protection was not utilized by employees while sheathing the roof.  b. On a 5:12 pitch roof of a temporary residential worksite, at 1711 McQuade Street, in Jacksonville, Florida 32209: On or about February 2, 2023, the employer exposed himself to fall hazards ranging from 8 feet to 15 feet 3-inches, in that fall protection was not utilized by the owner while sheathing the roof.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.   a. Left side of home, at a temporary residential worksite located at 1715 McQuade Street, Jacksonville, Florida 32209:  On or about February 3,  2023, the employer exposed three employees to fall hazards, in that, the 10-foot multi purpose extension ladder was not extended 3 feet above the landing.

29 CFR  1926.1053(b)(13): The top or top step of a stepladder was used as a step:  a. Inside the Garage, at a temporary residential worksite located at 1715 McQuade Street, Jacksonville, Florida 32209:  On or about February 3,  2023, the employer exposed employees to fall hazards, in that, employees used the top step of a Werner 6-foot A-frame ladder to exit from the roof, exposing themselves to fall hazards.
