At 3530 Dignan Street, Jacksonville, Florida 32254; On or about February 7, 2023, the employer exposed employees to head injuries in that hard hats was not utilized by employees while working underneath other employees on the roof installing sheathing.

29 CFR  1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  a. On the roof of a temporary residential worksite, at 3530 Dignan Street, Jacksonville, Florida 32254: On or about February 7, 2023, the employer exposed three employees to eye injury hazards caused by airborne debris and flying objects, in that eye protection was not utilized by the employees, while using pneumatic nail guns to sheath the roof.  b. On the roof of a temporary residential worksite, at 3530 Dignan Street, Jacksonville, Florida 32254: On or about February 7, 2023, the employer exposed two employees to eye injury hazards caused by airborne debris and flying objects, in that eye protection was not utilized by the employees while using saws to cut sheets of plywood and 2 x 4's.

On a 5:12 pitched roof of a temporary residential worksite, at 3530 Dignan Street, in Jacksonville, Florida 32254: On or about February 7, 2023, the employer exposed three employees to fall hazards ranging from 8 feet to 16 feet 3-inches, in that fall protection was not utilized by employees while sheathing the roof.

Inside the Garage, at a temporary residential worksite located at 3530 Dignan Street, Jacksonville, Florida 32254:  On or about February 7,  2023, the employer exposed employees to fall hazards, in that, employees used the top step of a Werner 4-foot A-frame ladder to exit from the roof, exposing themselves to fall hazards.  b. Inside the front room, at a temporary residential worksite located at 3530 Dignan Street, Jacksonville, Florida 32254: On or about February 7, 2023, the employer exposed employees to fall hazards, in that, employees used the top step of a Werner 6-foot-a-Frame ladder to exit from the roof, exposing themselves to fall hazards.
