29 CFR  1926.100(a):At this job site, employees exposed to possible head injury from falling or flying objects or from impact with hand tools, equipment and building materials during construction operations were not wearing hard hats:  a. At 1330 W 19th Street, Jacksonville, Florida 32209; On or about February 7, 2023, the employer exposed an employee to head injuries in that a hard hat was not utilized by the employee while working underneath other employees on the roof installing sheathing.

29 CFR  1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  a. On the roof of a temporary residential worksite, at 1330 W 19th Street, Jacksonville, Florida 32209: On or about February 7, 2023, the employer exposed three employees to eye injury hazards caused by airborne debris and flying objects, in that eye protection was not utilized by the employees, while using pneumatic nail guns to sheath the roof.  b. On the roof of a temporary residential worksite, at 1330 W 19th Street, Jacksonville, Florida 32209: On or about February 7, 2023, the employer exposed two employees to eye injury hazards caused by airborne debris and flying objects, in that eye protection was not utilized by the employees while using saws to cut sheets of plywood.

On a 4:12 pitched roof of a temporary residential worksite, located at 1330 W 19th Street, in Jacksonville, Florida 32209: On or about February 7, 2023, the employer exposed two employees to fall hazards ranging from 8 feet to 13 feet 6-inches, in that fall protection was not utilized by employees while sheathing the roof.

Right side of home, at a temporary residential worksite located at 1330 W 19th Street, Jacksonville, Florida 32209:  On or about February 7,  2023, the employer exposed an employee to fall hazards, in that, the employee used the top step of a Werner 6-foot A-frame ladder to have access to the roof, exposing himself to fall hazards.
