29 CFR  1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition:  a.) Palumbo's Meat of DuBois, Inc. - On or about 02/07/2023, and times prior thereto, miscellaneous cardboard boxes of sales products were stored in the walkways/aisleways with the upstairs walk-in freezer closest to the dumbwaiter. Items within walk-in freezer were not maintained in and orderly condition.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:  a.)  Palumbo's Meat of DuBois, Inc. - On or about 02/07/2023, and times prior thereto, employees were periodically exposed to a fall hazard of approximately 9.5-feet when working and walking next to an open dumbwaiter shaft in the upstairs area.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

Palumbo's Meat of DuBois, Inc. - On or about 02/07/2023, and times prior thereto, employees were potentially exposed to struck-by hazards in the upstairs freezer furthest from the dumbwaiter. Two cardboard boxes of product on the top shelf of the shelving unit were in danger of falling, in that they were on an angle leaning to the walkway.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

Palumbo's Meat of DuBois, Inc. - On or about 02/07/2023, and times prior thereto, employees periodically operated a Hyster forklift when relocating pallets of product inside the facility. Employees have not been trained and evaluated on forklift operations.
