29 CFR  1910.1030(c)(1)(iii):The employer did not ensure that a copy of the Exposure Control Plan was accessible to employees, in accordance with 29 CFR 1910.1020(e):  (a) Facility: On or about 1/31/2023, the employer exposed employees to biological hazards, in that a written exposure control plan was not at the site but was at the Fairburn location. The plan was not readily accessible to employees while performing tasks such as but not limited to phlebotomy.

29 CFR  1910.1030(f)(2)(i): Hepatitis B vaccination was not made available after the employee had received the training required in 29 CFR 1910.1030(g)(2)(vii)(I) or within 10 working days of initial assignment to employees who had occupational exposure::  (a) Facility: On or about 1/31/2023, and at times prior, the employer exposed employees to biological hazards, in that the hepatitis B vaccination or antibody testing for immunity was not provided to employees during their initial 10 days of occupational exposure while performing work activities such as but not limited to phlebotomy.
