29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a) New commercial construction located at 6371 Hwy 53 East, Dawsonville, GA 30534, Ground level: On or about 02/03/2023, and at times prior, the employer exposed employees to struck-by hazards while performing masonry-related tasks, in that the employer did not ensure that protective helmets were not in use while working/walking below the upper working surface.

29 CFR  1926.451(e)(1):When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  a) New commercial construction located at 6371 Hwy 53 East Dawsonville, GA 30534  scaffold system: On or about 02/03/2023, and at times prior, the employer exposed employees to fall hazards of approximately 14-feet to the lower ground level, in that an attached ladder was not provided at adequate heights to allow for safe access and egress from the scaffold system.

29 CFR  1926.501(b)(11):Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) New commercial structure located at 6371 Hwy 53 East, Dawsonville, GA 30534, Roofing level: On or about 02/03/2023, and at times prior, the employer exposed employees to fall hazards of approximately 14 feet to the lower ground level, in that a fall protection system was not provided to employees engaged in masonry activities on an 8 in 12 pitched roof.
