29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  a) On or about February 8, 2023, at the previously addressed inspection site, employees were roofing a residential building at heights greater than 6 feet above lower levels without fall protection, exposing the employees to falls.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  a) On or about February 8, 2023, at the previously addressed inspection site, employees exposed to falls while roofing a residential family home at heights greater than 6 feet above lower levels were not provided a training program to recognize the hazards of falls and how to minimize fall hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  a) On or about February 8, 2023, at the previously addressed inspection site, employees used a portable extension ladder to access the roof of a residential building with side rails that did not extend at least 3 feet above the roof surface exposing employees to falls.

The program shall enable each employee to recognize hazards related to ladders and stairways and shall train each employee in the procedures to be followed to minimize these hazards.  a) On or about February 8, 2023, at the previously addressed inspection site, employees using ladders with side rails that did not extend more than 3 feet above the landing surface to access the roof of a residential building were not provided training program to recognize hazards related to ladders and to minimize those hazards.
