29 CFR  1910.133(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a. On or about March 14, 2023, employees removing zinc surface dross were exposed to hazards related to zinc popping from temperature changes. Employees did not use face protection when removing zinc surface dross.

29 CFR  1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.  a.  On or about February 7, 2023, employees operating the Dynamex wire twisting machine #3 were exposed to caught-in hazards created by unguarded rotating parts such as, but limited to, the stem area spools and wheels, the payoff area pulleys and roll-ons, and the wires fed through the tape counter to be twisted in the Dynamex's ingoing nip-point.  Employees worked in close proximity to rotating parts designed to twist and wind insulated copper electrical wires while operating the machine..  b.  On or about February 7, 2023, employees operating Reeler machine #1 were exposed to caught-in hazards created by an ingoing nip point where armored cable wrapped around the reeler drum.  Employees operated the machine using a foot treadle to activate the reeler to pull the cable through the machine and coil it around the drum.  As the machine pulled the cable to the drum, the operator held the cable in hand, covered in cloth, on the top of the wires to check for snags within arm's reach of the unguarded nip point.  c.  On or about February 7, 2023, employees operating the unwinder machines behind SAM #20 were exposed to hazards created by unguarded rotating parts. Unwinder machines moved continuously at floor level on employee walking and working paths, which exposed employees to trip and fall hazards.  d.  On or about February 7, 2023, employees operating the unwinder machines behind SAM #12 were exposed to hazards created by unguarded rotating parts. Unwinder machines moved continuously at floor level on employee walking and working paths, which exposed employees to trip and fall hazards.

29 CFR  1910.212(a)(3)(ii):The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.  a.  On or about February 7, 2023, employees operating SAM 12 machine were exposed to the point of operation.  Plastic shields covering the point of operation could be opened, lifted, and/or moved while the machine ran, which exposed employees to caught-in hazards.  b. On or about February 7, 2023, employees operating SAM 20 machine were exposed to the point of operation.  Plastic shields covering the point of operation could be opened, lifted, and/or moved while the machine ran, which exposed employees to caught-in hazards.  c. On or about February 7, 2023, employees operating SAM 36 machine were exposed to the point of operation.  Plastic shields covering the point of operation could be opened, lifted, and/or moved while the machine ran, which exposed employees to caught-in hazards.  d.  On or about March 14, 2023, employees operating SAM 11 machine were exposed to the point of operation.  Plastic shields covering the point of operation could be opened, lifted, and/or moved while the machine ran, which exposed employees to caught-in hazards.  e.  On or about March 14, 2023, employees operating SAM 19 machine were exposed to the point of operation.  Plastic shields covering the point of operation could be opened, lifted, and/or moved while the machine ran, which exposed employees to caught-in hazards.  f.  On or about March 14, 2023, employees operating SAM 23 machine were exposed to the point of operation. Guards shall be affixed to the machine where possible and secured elsewhere if for any reason attachment to the machine is not possible. The guard shall be such that it does not offer an accident hazard in itself.  a.  On or about February 7, 2023, employees operating SAM 12 machine were exposed to the point of operation.  Plastic guarding was not securely affixed to the exterior, which exposed employees to caught-in hazards.  b.
