The employees were working on all areas of the roof and the employer did not ensure that they were using fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about February 8, 2023, employees were exposed to a fall of approximately 12 feet while performing roofing activities on a residential home located at 34080 Harvest Point Drive, North Ridgeville, Ohio 44039. The employees were using an extension ladder to access the roof and the employer did not ensure it extended three feet above the landing area.
