29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section.  a) On or about 02/09/2023, in the Formica Area; Lockout procedures were not utilized when employees were doing servicing or maintenance such as, but not limited to: Boring machines, changing blades on table saws, exposing employees to struck-by and amputation hazards.   b) On or about 02/09/2023, in the Punch Press Area; Lockout procedures were not utilized when employees were removing/replacing dies which would protect the press operators from the downward motion of the ram of the mechanical power press such as, but not limited to: Cincinnati #21, Niagara #20, Chicago #24, Punch press #20. Employees were exposed to crushing/amputation hazards.  ABATEMENT DOCUMENTATION REQUIRED
