29 CFR 1910.132(d)(1):The employer did not assess the workplace to determine if hazards were present, or were likely to be present which necessitated the use of personal protective equipment (PPE): The employer is failing to protect employees from exposure to crushing and smashing hazards. This was most recently documented on February 10, 2023 when the employer did not conduct a job hazard analysis assessment of the new task at a new temporary workspace. Employees working outside at the Southeast door were exposed to crushing and smashing hazards where an employee's left hand was crushed underneath a flat metal sheet that weighed about 3,000 pounds, while he was handling it with another employee. Employees moving/handling metal using two forklifts were permitted to work in a temporary area with a narrow overhead door clearance that would not fit through the door with the big, heavy, long piece of Aluminum plate, without assessing the new workplace to determine if hazards were present.
