29 CFR 1926.153(h)(13): Blower or radiant type heaters were directed towards an LP-Gas container located within 20 feet of the blower or heater: (a)Michael Johnson Contracting, Inc at 2200 South Daylight Drive, Sioux Falls, SD 57110: On or about and at times prior to February 10, 2023, the employer did not ensure that the L.B. Premier 80 DF 2.0 Direct Fired Heater was not directed towards the LP-Gas tank suppling the heater. This condition exposed employees to fire and explosion hazards.

29 CFR 1926.451(c)(2)(i): Footings were not level, sound, rigid, and capable of supporting the loaded scaffold without settling or displacement: (a)Michael Johnson Contracting, Inc at 2200 South Daylight Drive, Sioux Falls, SD 57110: On or about and at times prior to February 10, 2023, the employer did not ensure that scaffolding footings were adequately supported due to unstable footings bearing on unstable objects while performing masonry operations from a fabricated scaffold. This condition exposed employees to crushing hazards.
